Passport Applications

Please carefully review this information before booking an appointment. It is required that your application be completed before your appointment. If you need assistance with your forms, please arrive 30 minutes early.

Please note that we do not process renewals with a DS 82 form. However, we can assist you with a photo for this form.

Bring evidence of U.S. Citizenship

  • Original Birth Certificate
  • Naturalization Certificate

Bring a government Issued Photo ID

  • Valid U.S. Driver’s License (out of state of FL requires a second ID)
  • Valid Military ID
  • Valid Passport

Other Important Information

  • Photos are taken at our office for $15.00.
  • Other required documents may include: marriage licenses, dissolution of marriage and custody judgments. These will need to be certified copies.
  • Two separate payments: First, U.S. Department of State application fees must be paid by check or money order. Second, a separate Clerk of Court processing fee can be paid by cash, check, money order or credit/debit (with a 3.5% processing fee.)

U.S. Department of State Fees:

  • Adult Passport Book Fee: $130
  • Adult Passport Card Fee: $30
  • Child Passport Book Fee: $100
  • Child Passport Card Fee: $15
  • Expedited fees: $60 per application
  • Express Return Mail Fee (Book Only): $21.36

Clerk of Court Fees:

  • Processing Fee: $35 per application
  • Photo Fee: $15.00 each
  • Express Mail to the U.S. Department of State: $31.40

Please note the Clerk of Court Processing Fees and the U.S. Department of State Fees are separate.

  • The U.S. Department of State requires a check or money order only.
  • The Clerk can take cash, check/money order or debit/credit for payment. Please note if using debit/credit for the Clerk’s payment, a fee of 3.5% will be charged.

U.S. Department of State Fees

Payment to the U.S. Department of State is in check or money order only, made payable to the U.S. Department of State. A separate check per application is required.
